Effective May 1, 2026, all requests—including permit orders, validations, code requests, carpool applications, and departmental refund requests—must be submitted exclusively through an online FireForm. Paper forms and previously available PDF forms will no longer be accepted after this date.
If you would like to return your permit for a refund, please complete the Refund Request Form. Approved refunds will be issued to the original form of payment. Please note that completion of a refund form does not guarantee a refund will be issued to you.
IU Indianapolis or IUH departments needing parking permits should complete the Permit Order Form. We will process your order and let you know when your order is ready for pickup. Your order will be processed within 7 to 10 business days.
IU Indianapolis and IU Health departments/schools can request validation tickets to cover all or a portion of your guest’s parking.
Departments requesting validation tickets for the very first time will need to request a validation number. Fill out the Request for New Parking Validation Form to create a validation number unique to your department.
Note: You will only need to complete this form for the initial setup. A separate form will need to be completed if another validation number needs to be created for a different account number.
Have an existing validation number? Need to order more tickets? Simply complete the Validation Ticket Order Form to place an order. Validation ticket orders will be processed within 10 business days. Please be aware that a fee of $75.00 will be charged for all rush orders.
IU Indianapolis departments/schools are able to request garage entry/exit codes to pay for your visitor's parking by filling out the Code Order Form.
Applicants must be IU employees and complete this form to be considered for a carpool permit.
